Confidentiality of records and disclosures. Except when required by law, you have the right to approve or refuse the release of records.


The opportunity to participate in decisions involving your health care, including the right to accept or refuse medical or surgical treatment, unless contraindicated by concerns for your health.


Change primary or specialty physician or dentist if others are available.


Information concerning implementation of any advance care directive.


Impartial access to treatment regardless of race, color, sex, national origin, religion, handicap or disability. (The Surgery Center adheres to all Federal and State rules, regulations, and policies to promote a safe and non-discriminatory environment for all of our guests).


Request information on fees for services or payment policies and receive a bill for services upon request.


Know the identity and professional status of individuals providing service to you.


Report any comments concerning the quality of services provided to you during the time spent at the facility and receive fair follow-up on your comments, complaints or grievances without discrimination or reprisal.


If adjudged incompetent under applicable state health and safety laws by a court of proper jurisdiction, your rights will be exercised by the person appointed under state law to act on your behalf.


If not adjudged incompetent, any legal representative or surrogate designated by you in accordance with state laws may exercise your rights to the extent allowed by state law.


Exercise any and all of these rights without being subjected to discrimination or reprisal.

YOUR RESPONSIBILITIES

Providing to the best of your knowledge, accurate and complete information about your present health status and past medical history and reporting any unexpected changes to the appropriate practitioner(s).


Following the treatment plan recommended by the primary practitioner involved in your case.


Providing an adult to transport you home after surgery and an adult to be responsible for you at home for the first twenty-four (24) hours after surgery.
Indicating whether you clearly understand a contemplated course of action and what is expected of you.


Your actions if you refuse treatment, leave the facility against the advice of the practitioner, and/or do not follow the practitioner’s instructions relating to your case.


Assuring that the financial obligations of your health care are fulfilled as expediently as possible.


Providing information about and/or copies of any living will, power of attorney or other directives that you desire us to know about.
Being respectful of all healthcare providers and staff as well as other patients.

SUBMISSION AND INVESTIGATION OF GRIEVANCES

If you have any concerns regarding your rights and responsibilities, or if you have any complaints or grievances on how these rights were or were not administered, please contact the Civil Rights Coordinator who will investigate your issue.

All Cosmetic fees and copays are due at the time of service. Copays not paid at the time of service are subject to a $25.00 late fee.

Coupons and Gift Certificates must be presented at time of service to be valid.

Reminder to insurance patients: A coverage determination, prior authorization, or certification that is made prior to a service being performed is not a promise to pay for the service at any particular rate or amount. The patient’s summary plan description governs amount payable, as every claim submitted is subject to all plan provisions, including, but not limited to, eligibility requirements, exclusions, limitations, and applicable state mandates.

In order that Northwest Face may provide all clients with the very best of care, please respect the following policies:

  • Please maintain conversations at a considerate volume
  • Please turn off all pagers and cell phones
  • No pets, please
  • All minors must be accompanied by an adult

LATE ARRIVALS AND CANCELLING

Appointments have been designed to allow the appropriate time for the extent of your service. Your late arrival may limit our ability to offer the fullest experience possible.

We understand that sometimes it is necessary to re-schedule appointments. We ask that you give at least 24 hours notice prior to canceling and we will gladly reschedule your appointment. In the event that you are unable to give sufficient notice, a $50 cancellation fee may apply. We regret any inconvenience this may cause.

GIFT CARDS

Gift Cards are a memorable expression of caring, from the unique presentation to the exquisite experience. Cards are available in any denomination and may be applied towards all products and services, including cosmetic surgery.

If you are a recipient of a donated or auctioned Gift Certificate, please give our receptionist the certificate number to book your appointment. Donation certificates are non-transferable, and their expiration dates may not be changed or extended.

Parking is available in self-paid lots. We regret that we are unable to validate parking.

PRIOR TO SURGERY

All patients must arrange for a responsible adult driver (over the age of 18) to remain in the Surgery Center during the entire stay. Your driver must have a valid driver’s license. Failure to have a driver could result in delay or cancellation of your procedure; taxi cab, bus, and other forms of public transportation are only acceptable only if you have adult companion with you.

You may receive a phone call from a pre-admission nurse. You will be asked questions about the medications you take and their dosages, your medical history, and your surgical history. Please follow your physician’s instructions if you received a pre-admission phone call from the Surgery Center.

If your situation involves legal guardianship, please contact the Center prior to your procedure to prevent any delays in your procedure. Failure to provide guardianship paperwork and/or have the legal guardian present could lead to delay or cancellation of your procedure.

Call the Surgery Center at (231) 392-8900 the day before your scheduled procedure between the hours of 1:00 PM and 5:00 PM to receive your arrival time. If your procedure is on a Monday, please call the Surgery Center the Friday prior to your procedure for your arrival time. If you have received an arrival time from your physician’s office, you do not have to call the Surgery Center.

If you should need to cancel or reschedule your procedure, please notify your surgeon’s office to make those arrangements.

IMPORTANT POINTS TO REMEMBER

DO NOT drink alcohol 24 hours prior to your procedure.


Stop smoking as early as possible before your procedure and for the duration of your recovery.


If you have Advance Directives, please let your surgeon know so that you can discuss those with him or her prior to your visit.


Contact your physician if…

(1) you have questions about which medications you can take prior to your procedure.

(2) Develop a cold, persistent cough, fever, or any other health-related concern prior to your procedure. follow your physician’s instructions.

(3) Need to reschedule or cancel your procedure.

THE NIGHT BEFORE YOUR PROCEDURE

Please DO NOT eat or drink anything after midnight, including: water, juice, coffee, chewing gum, or mints/ hard candy unless you have been otherwise instructed by your physician. If you must take medications prior to your arrival, take them with the smallest sip of water.

Failure to follow this instruction could lead to cancellation or postponement of your procedure.

ON DAY OF SURGERY PLEASE:

DO NOT wear make-up, body lotion, or nail polish.
DO remove all valuables (jewelry, body piercings, etc.) and leave at home.  CRSC does not accept financial responsibility for lost valuables or personal belongings.
DO wear glasses; if you must wear your contacts, please bring a container for your lenses.
DO wear loose, comfortable clothing that you can change easily and that will not bind at the site of your surgery/procedure.
DO take a shower or bath the morning of surgery or the night before

PLEASE BRING:

A responsible adult over the age of 18 with a valid driver’s license to wait at the Center while you have your procedure and drive you home. CPSC has free Wi-Fi available and a café in the lobby. Please also bring your own photo ID.


A list of all medications that you are currently taking, including dosages and frequency. Please include vitamins, supplements, and herbal remedies.
Heart and/or blood pressure medication(s) in their original labeled containers.


Advance Directive, Healthcare Power of Attorney, and/or applicable guardianship papers.


Any items issued/prescribed by your physician for your procedure, such as: crutches, immobilizer/brace, and/or ice machine. (Please leave crutches in the car; any brace, immobilizer, and/or ice machine should be brought with you into the Center.)
Oxygen tank, inhaler(s), or CPAP machine if you use these items.


Medical device identification if you have a pacemaker or defibrillator.


Your remote programmer if you have an implanted electrical stimulating device.

PRIOR TO SURGERY

When you arrive, you will be greeted by the Front Desk staff, who will register you, ensuring all of your personal information is correct.


You will be taken to the pre-operative area where a nurse will review your personal information, health history, and medications/allergies with you. They will also prepare you for your procedure or surgery. Your driver can come with you to the pre-op area if you wish.


Your physician, and possibly an anesthesiologist, will talk with you next; allowing you to ask any questions before your procedure or surgery.

LEAVING THE SURGERY CENTER

For most procedures you will be ready to leave the Surgery Center within 2 hours following completion of your surgery.


You must have a responsible adult with you to drive you home or accompany you if you plan to use a taxi service. Use of a taxi or public transport service will not be permitted without an accompanying adult.


DO have someone plan to stay with you through the night.


DO NOT drive or return to work on the day of surgery.

WHEN YOU GET HOME

DO follow your discharge and post-operative instructions carefully.


DO NOT drive or operate machinery, sign legal documents, drink alcoholic beverages, or use recreational drugs for at least 24 hours after surgery.


Patients often experience minor effects following anesthesia, such as drowsiness, nausea, muscle aches, a sore throat, headaches, or dizziness. These side effects decline in the hours following your procedure but may take several days before they are gone completely.


The majority of patients will feel up to their typical activities the next day; but remember as you go about your day to move slowly and carefully and to get plenty of rest.


It is common to experience constipation after surgery. The most common causes of constipation after surgery are pain medications and anesthesia.


Contact your physician if you have questions about what you can and cannot do, when you can return to work, and how active you may be.


If you experience unexpected pain, severe bleeding, or a temperature greater than 101 degrees Fahrenheit, have your physician paged by calling (123) 123-4567 or immediately proceed to the hospital emergency room.
